Intermountain Health and CareCentra have broadened their collaboration to address chronic obstructive pulmonary disease (COPD) and adult asthma more effectively by introducing remote monitoring technologies to detect symptom exacerbation early.
Under this extended partnership, CareCentra’s prevention-as-a-service platform will employ advanced algorithms to monitor various data points and biometrics from digital devices, combined with patient-reported information, to identify signs of risk.
The AI-driven system will guide patients on proper inhalation techniques, notify them of environmental hazards, aid in medication adherence, and encourage lifestyle changes. If the AI detects significant risk, it alerts Intermountain Health’s Pulmonary Disease Navigator team for immediate intervention and coordination with primary care providers.
Intermountain Health aims to expand its successful Hospital Readmission Reduction Program (HRRP) by leveraging CareCentra’s platform to monitor the approximately 300,000 COPD and adult asthma patients within its network.
CareCentra’s CEO, Vasant Kumar, emphasized the integration of digital health and AI to manage patient risks effectively. Kim Bennion, from Intermountain Health, highlighted the platform’s ability to complement human care efforts efficiently.
