KIMS Hospitals Performs Karnataka’s First NitrX Medial Pivot Total Knee Replacement with Kinematic Alignment

KIMS Hospitals, Mahadevapura, has successfully performed Karnataka’s first NitrX Medial Pivot total knee replacement surgery using the Kinematic Alignment Instrument Set. A specialised orthopaedic team led by Dr. Kumardev Arvind Rajamanya carried out the procedure as part of the hospital’s advanced orthopaedic services.

This milestone highlights the growing adoption of next-generation knee replacement techniques designed to replicate the natural movement of the knee joint more accurately.

Patient with Severe Osteoarthritis Seeks Mobility

As per the press release, the surgical team performed the procedure on a 65-year-old woman diagnosed with Grade 4 osteoarthritis, the most advanced stage of the disease. Patients at this stage typically experience severe pain, joint deformity, and instability while walking.

Despite these challenges, the patient maintained an active lifestyle and regularly climbed stairs. Therefore, she sought a treatment option that would relieve her pain while helping her maintain mobility and independence.

Following evaluation, doctors recommended a Medial Pivot knee replacement with kinematic alignment, a technique designed to closely restore the patient’s natural knee mechanics.

Successful Surgery and Quick Recovery

The medical team completed the surgery successfully, and the patient showed rapid recovery after the procedure. Within just three days, doctors discharged her from the hospital, demonstrating the effectiveness of the advanced surgical approach.

How the Medial Pivot Design Improves Knee Function

Unlike conventional knee implants, the Medial Pivot design closely replicates the natural biomechanics of the human knee.

Specifically, the implant provides greater stability on the inner (medial) side of the knee, functioning similarly to a ball-and-socket joint. At the same time, it allows controlled forward and backward movement on the outer (lateral) side, which mimics the natural motion of a healthy knee.

Additionally, the NitrX implant features a titanium niobium nitride coating, which reduces metal reactivity. This characteristic makes it particularly beneficial for patients with metal sensitivities.

Furthermore, the implant uses a bone-conserving, single-radius design, which helps maintain consistent ligament tension throughout the full range of knee movement, improving stability and comfort.

Surgeon Explains the Surgical Approach

Commenting on the case, Dr. Kumardev Arvind Rajamanya, Director & Lead Consultant – Orthopaedic Surgery & Complex Trauma, Robotic Joint Reconstruction / Replacement, Sports Medicine & Arthroscopy, explained the rationale behind the procedure.

“This patient had severe Grade 4 osteoarthritis with deformity and instability. However, she remained very active and wanted to continue climbing stairs comfortably. Therefore, we chose a procedure designed to restore knee function in a way that closely replicates her natural anatomy.

The Medial Pivot design provides stability on the medial side while allowing controlled lateral translation, which closely resembles the movement of a natural knee. In addition, with kinematic alignment, we recreate the patient’s natural pre-arthritic knee alignment rather than applying a uniform alignment used for all patients.”

Commitment to Advanced Orthopaedic Care

Dr. Nitish Shetty, Managing Director, KIMS Hospitals (Krishna Institute of Medical Sciences), Bengaluru, highlighted the hospital’s focus on adopting innovative and patient-focused surgical approaches.

Growing Adoption of Precision-Based Orthopaedic Techniques

This development reflects the increasing adoption of advanced, bone-conserving, and anatomically accurate knee replacement procedures in India. Such innovations emphasize the importance of precision-based and outcome-driven orthopaedic care, helping patients regain mobility and improve their quality of life.
