Apollo HomeCare, Apollo AyurVAID Partner to Improve Post-Discharge Recovery

Apollo HomeCare and Apollo AyurVAID have partnered to address the critical gap patients often face between hospital discharge and safely returning home. Together, they have launched an integrated Transition Care service that offers clinically monitored, in-facility step-down care for patients recovering from major treatments and procedures.

Through this initiative, patients receive continuous assessment, stabilisation, and recovery support before they transition back home. The programme combines modern medicine with Ayurveda to create a more structured and comprehensive recovery pathway.

Clinically Monitored Step-Down Care Ecosystem

The Transition Care facilities function as an intermediate care ecosystem positioned between the hospital and home. Here, patients continue to receive essential medical and recovery support, including nursing care, physiotherapy, rehabilitation, nutritional counselling, and selective Ayurvedic therapies tailored to their stage of recovery.

Moreover, doctors and care teams from both Apollo HomeCare and Apollo AyurVAID closely monitor each patient’s condition and respond quickly to any changes. The primary objective remains clear: ensuring that patients become stable, stronger, and fully prepared to return home safely.
