Jupiter Hospital, Thane, recently treated a rare and challenging case of NMDA receptor positive autoimmune encephalitis in a 73-year-old male patient. This condition, typically observed in young women with ovarian teratomas, is extremely uncommon in elderly males — making this case a remarkable milestone in clinical expertise and patient care.
Complex Patient Presentation
As per the press release, the patient, with a history of diabetes mellitus (DM), hypertension (HTN), and carcinoma bladder (post-TURP), presented with severe neurological symptoms. He experienced headache, confusion, drowsiness, and orofacial dyskinetic movements.
Initial CT and MRI scans ruled out acute infarct or intracranial bleed, prompting further investigations. Cerebrospinal fluid (CSF) and serum tests confirmed NMDA receptor positive autoimmune encephalitis.
Intensive, Multidisciplinary Treatment
Doctors initiated an aggressive treatment protocol with high-dose steroids (1 gm Solumedrol IV for three days), followed by five cycles of Intravenous Immunoglobulin (IVIG) therapy.
A multidisciplinary team — including specialists from neurology, endocrinology, psychiatry, and critical care — worked in close coordination to stabilize the patient and manage his multiple comorbidities.
Expert Insights
“NMDA receptor encephalitis is rare in elderly males and requires a high degree of clinical suspicion for early diagnosis. Prompt initiation of steroids and IVIG therapy can be life-saving, as seen in this case. The patient responded well to treatment and is now on the path to recovery,” said Dr. Yogesh Godge, Consultant Neurologist, Jupiter Hospital, Thane.
Highlighting the importance of an integrated approach, Dr. Javal Bhatt, Consultant, Internal Medicine, Jupiter Hospital, Thane, added, “Managing such a complex case required balancing the patient’s comorbidities while targeting the underlying autoimmune process. Timely coordination between departments was crucial for achieving this positive outcome.”
Remarkable Recovery and Hospital Commitment
Following the treatment, the patient showed significant improvement — regaining consciousness, becoming responsive, and following commands. This marked the start of a successful recovery despite the rarity and severity of the condition.
This case underscores Jupiter Hospital’s advanced neuro-care capabilities and its dedication to managing rare, life-threatening conditions through a comprehensive, team-based approach.
