A Landmark Achievement in Complex Cancer Surgery
Doctors at KIMS Hospitals, Kondapur, successfully performed a rare and highly complex surgical procedure on a patient who had previously undergone a kidney transplant and was later diagnosed with pancreatic cancer. Notably, the surgery is believed to be the first in the world to be performed using a laparoscopic approach for such a medically challenging case.
Diagnosis During Post-Transplant Follow-Up
According to Dr. Naveen Kumar CH, Consultant Surgical Gastroenterologist at KIMS Hospitals, Kondapur, the patient had undergone a kidney transplant earlier and was on three types of immunosuppressive medications to prevent organ rejection. During a routine follow-up evaluation, doctors detected pancreatic cancer, which required immediate and carefully planned surgical intervention.
Choosing a Minimally Invasive Surgical Approach
After evaluating the patient’s condition, the medical team decided to perform a laparoscopic Pancreatoduodenectomy, commonly known as Whipple’s surgery. However, the procedure posed multiple technical challenges.
In addition to the transplanted kidney, the patient also had polycystic liver disease and a large cyst in the transplanted kidney. Furthermore, the patient had previously undergone open bilateral nephrectomy, which added complexity to the surgical procedure.
Managing Critical Vascular Involvement
Another major challenge involved the Superior Mesenteric Vein (SMV), a vital blood vessel that supplies the intestines and liver. During the surgery, the team carefully performed laparoscopic Whipple’s surgery while simultaneously resecting and repairing the affected segment of the SMV.
As per the press release, Dr. Naveen Kumar explained that laparoscopic vascular resections are performed in only a few specialized centres worldwide. Moreover, performing major pancreatic surgery in a patient on long-term immunosuppressive therapy carries extremely high risks, with reported mortality rates reaching up to 50 percent in certain cases. Therefore, the surgical team ensured meticulous intraoperative management and intensive post-operative care to minimize complications.
A First-of-Its-Kind Laparoscopic Procedure
Dr. Kumar further noted that a similar case had previously been reported at the Mayo Clinic in the United States. However, surgeons performed that procedure using a conventional open surgical technique.
“In contrast, this is the first time such a complex procedure has been successfully carried out using a laparoscopic technique,” he said.
