Doctors at Iswarya Hospital, OMR, Chennai successfully diagnosed and treated a 28-year-old man who suffered an acute ischemic stroke despite showing none of the typical warning signs. The case highlights the importance of recognizing subtle stroke symptoms and seeking prompt medical attention.
The patient, a young smoker who had recently returned from Singapore, experienced an unusual sensation on the left side of his body. However, he did not have common stroke symptoms such as weakness, facial drooping, slurred speech, difficulty walking, or impaired limb movement.
Clinical Vigilance Leads to Early Detection
The patient initially consulted a physician in Chennai, where a routine examination found no obvious abnormalities. As the strange sensation persisted, he visited Iswarya Hospital, OMR, in April 2026, where Dr. Deepa Avadhani, Senior Consultant Neurologist, conducted a detailed evaluation.
Although his neurological examination appeared normal, Dr. Deepa recommended an urgent MRI scan. The scan revealed an acute right thalamic infarct, a type of ischemic stroke affecting the brain’s sensory processing centre.
As per the press release, doctors immediately started the patient on antiplatelet therapy, statins, and neuroprotective medications. Within a week, his symptoms improved significantly, and he was discharged with complete neurological recovery.
Stroke Can Present Without Typical Symptoms
Commenting on the case, Dr. Deepa Avadhani, Senior Consultant Neurologist at Iswarya Hospital, said, “Stroke is commonly linked to symptoms such as paralysis, slurred speech, or collapse. However, this case shows that it can also present very subtly. In this patient, the stroke affected the thalamus, which is responsible for sensory perception rather than movement or speech. As a result, his only symptom was an unusual altered sensation that could easily have been overlooked.”
She further explained that the thalamus serves as the brain’s sensory relay centre, processing touch, temperature, and body awareness. Consequently, a stroke in this region may cause abnormal sensations without any visible weakness, making diagnosis more challenging.
